VPIS Poisons Checklist

A Tool to Assist with History Taking

Taking a case history over the telephone from a worried owner can sometimes be difficult, especially if the animal is already unwell.  Although there may be some urgency to seek advice from the VPIS, it is important to ask the owner for as much relevant case information as possible.  The VPIS offers poisons information tailored to your individual case, and the advice we give is largely dependent on the original history provided.  To help you collect the information we require, we have created the Poisons Checklist (see sample page below).

Click here to download a PDF of the Poisons Checklist.
Why not print a few out, keep them by your telephone and use them to help take a case history when you receive a poisons telephone enquiry.

If you have any suggestions for improvements for future versions of the checklist please email vpis@gstt.nhs.uk
